Who this covers
This policy explains how Mohammed Ismail ("I", "me") collects and uses information through mohammedyismail.com. It covers the website only. Work done for clients is governed by the client's contract, including any Business Associate Agreement.
What I collect
- Information you give me: your name, work email, practice name, website, role and answers to the Readiness Score or contact form.
- Usage data: basic, aggregated information about how the site is used, such as pages viewed. If analytics are used, they are configured to avoid storing personal identifiers where possible.
This website is not designed to collect health information. Please don't enter any patient information or personal health details in any form.
How I use it
- To send you the report or reply you asked for.
- To follow up about your request.
- To send occasional practice-growth notes, only if you opted in. You can unsubscribe at any time.
- To improve the website.
Who I share it with
I use service providers to run this site, send email and manage contacts, such as hosting, email delivery and CRM software. They process data on my behalf. I don't sell your information and I don't share it with advertisers.
How long I keep it
Contact and Readiness Score information is kept for as long as it is useful for our conversation, and no longer than 24 months after our last contact, unless you become a client.
Your choices
You can ask to see, correct or delete your information at any time by emailing contact@mohammedyismail.com. Depending on where you live, you may have additional rights under local privacy laws.
Security
The site uses HTTPS, and access to stored information is limited and protected with multi-factor authentication.
Changes
If this policy changes, the date at the top of the page will change too.
